When you request treatment and I agree to provide that treatment we enter into a contract. The information you provide is entirely your choice.

The information I ask you to share with me for the process of Psychotherapy/Counselling is in order to provide you with the best possible approach to your therapeutic needs.

Collecting information allows me to do my job effectively. I can only use this information for the purpose for which it was recorded, that is, the provision of counselling or psychotherapy services.

During our first session I will take notes of what you and I discussed. In the following sessions I will keep minimum notes of our sessions.

Why I keep session notes:

  • To document and review the counselling and psychotherapy services provided.
  • As a protection from professional liability. I am legally required to keep client records for a period of 10 years. This is in line with the professional guidelines of IAHIP and IACP. After that period, I will dispose of all records in a safe and secure manner.
  • I keep your phone number in case I need to contact you in the event of a change to an appointment time

Data Storage:

  • All digital data will be encrypted.
  • All notes are anonymised according to GDPR guidelines.
  • I will keep your phone number on my phone for the duration of our work together and they will be removed upon termination. Your number will appear under your initials only. My phone is password protected.
  • I will never share any data with a third party unless data are requested because a person is involved in legal proceedings or because of Child Protection concerns.

You have the right to access the personal data of yours I hold by putting a request in writing to me via Subject Access Request (SAR). You also have the right to ask the correction of factual errors.

If you have any concerns on any of the above you have the right to let me know. If you are not satisfied with the response you have the right to raise the matter with the Data Protection Commissioner’s Office.
